Three free printable PDFs for adults: an assessment preparation sheet, a 7-day symptom diary and a guide for a partner or relative. Not diagnostic tools.

These are three free PDFs you can print before an adult ADHD assessment: a preparation sheet, a 7-day symptom diary and a guide for a partner or relative. They are not diagnostic tools and do not replace a clinical assessment.

An assessment rests on concrete examples and on your history since childhood. NICE guidance asks for a full developmental and psychiatric history and, where possible, information from someone who knows you well [1]. These sheets help you gather that material before the session, so the time with the doctor goes to understanding it.

The worksheets are currently available in Greek only.

1. Preparation sheet for the assessment

Seven prompts to organise what you want to say: examples of everyday difficulties, childhood, medical history and your questions for the doctor. Two A4 pages, to complete before the first session.

Write examples, not verdicts. “I am always late” is a conclusion. “Last Tuesday I missed the 9:00 meeting because I started an email at 8:30” is something we can work with. If you catch yourself writing “always” or “never”, ask: is that true every time, or only the times I remember best?

For the childhood prompt, think of school in concrete terms: your grades, what teachers said or wrote about you, and how studying went. A note such as “I couldn’t stay in my seat for a whole lesson” is more useful than “I was a bit distracted”.

2. Seven-day symptom diary

One landscape A4 page, a table for concentration, unfinished tasks, impulsive decisions, sleep and emotional intensity over one week. It is not a diagnostic tool.

Action comes before mood. Fill in the first line today, even if you do not feel like it, because waiting until you feel motivated is how a week of notes turns into none. Short entries are enough. A few honest lines each day are worth more than one perfect page that never gets written.

If work fills most of your week, add the hours you worked each day. Long hours can be a way of keeping up, and sometimes a way of proving your worth (“if I don’t work hard, what am I worth?”). Seeing the real number on paper is the first step towards setting a limit.

3. Guide for a partner or relative

One A4 page for someone who knows you well: how to describe to the doctor what they observe, with specific examples, what to avoid and how to help afterwards.

Handing this over is not an accusation. It is a way to add a second view of the same situation.

This second view carries real weight. A definitive diagnosis waits for confirmation from a third person, ideally a parent who remembers your childhood, or a partner. If that is not possible, the clinical interview decides, so not having someone to ask does not stop the assessment.

What next?

None of this is a test you can fail. The sheets only collect information. If the difficulties regularly get in the way of work, relationships or daily functioning, an assessment is worth considering. For a first check, try the free ASRS screener, which shows whether an assessment makes sense and does not diagnose. See also how to prepare, the adult ADHD service, or book an appointment.

Educational information. It does not replace an individual clinical assessment.
